This page includes information and tools to support HMRF grant recipients in using nFORM and conducting continuous quality improvement (CQI). If you have any questions after reviewing these resources, you may request technical assistance by emailing nFORMCQITA@mathematica-mpr.com.
Please contact your grant organization's nFORM site administrator(s) for help with log in issues including resetting passwords and reactivating accounts. Site administrators should leave a voicemail for Mathematica at 844-619-6841 if they are also unable to log into nFORM.

This is PDF of the PPR, including the quantitative data tables and qualitative progress narrative questions. Grant recipients must access and complete the PPR from the Reports tab in nFORM. Once the report has been completed, upload the final PDF into Grant Solutions.

The tip sheet also includes a matrix that presents advantages and disadvantages of methods for identifying bright spots, a collaborative activity to identify success factors with staff, and an interview template to learn more about practices used by bright spots.
