nFORM and CQI Resources

This page includes information and tools to support HMRF grant recipients in using nFORM and conducting continuous quality improvement (CQI). If you have any questions after reviewing these resources, you may request technical assistance by emailing nFORMCQITA@mathematica-mpr.com.

Please contact your grant organization's nFORM site administrator(s) for help with log in issues including resetting passwords and reactivating accounts. Site administrators should leave a voicemail for Mathematica at 844-619-6841 if they are also unable to log into nFORM.

  • PPR and Progress Narrative Template

    This is PDF of the PPR, including the quantitative data tables and qualitative progress narrative questions. Grant recipients must access and complete the PPR from the Reports tab in nFORM. Once the report has been completed, upload the final PDF into Grant Solutions.

  • nFORM and CQI Training #3: Capturing Workshops and Individual Services in nFORM

    This training session covers managing client profiles, setting up workshop series, recording workshop attendance, capturing individual service contacts, tracking referrals to outside services, and documenting incentives and program supports. The data capacity and CQI TTA team also answers questions from grant staff.
