The Privacy Act Statement is a federally required notice that explains to clients why HMRF programs collect their information, how it will be used, how it will be protected, and what rights they have regarding participation. It helps ensure that clients understand that providing information is voluntary, that they may choose not to answer specific questions, and that their information will be handled in accordance with federal privacy protections. Grant staff must share the privacy statement with potential applicants during outreach and enrollment and before administering client surveys so that each applicant and client is aware of their data privacy rights. This statement, available in nFORM and in the link below, must be shared before grant staff can enter applicant or client information into nFORM. Grant recipient staff must then present the Privacy Act Statement to the applicant exactly as written in the application form, either verbally or in writing. In nFORM, staff must check the box indicating that the client has been offered the privacy statement in order to save the form. Note that grant programs must provide the Privacy Act Statement to applicants who are ready to enroll even if the applicant was provided the statement during outreach.
