Grantees may use this resource as a reference for using nFORM 2.0 data for program operations, monitoring, and improvement. This resource also includes which tabs of the data dictionary to reference when reviewing the data.

Program staff at all levels can use this worksheet to document questions they have about their HMRF grant work. After discussing those questions with their HMRF data manager, they should record the source(s) of information and answers, and identify next steps.

The data dictionary provides the record layout for the data fields that are included in the data export from nFORM. nFORM users should refer to the applicable version of the data dictionary for data exports generated through November 30, 2022, those generated between December 1, 2022 and August 5, 2024, and those generated on or after August 6, 2024.
